Open Records Request

Use this form to request governmental records. Describe the exact information you are requesting and include details that may help locate the information. The city is not obligated to create a document to satisfy a request. If the requested record contains information that is confidential or exempt from public disclosure in accordance with the Texas Public Information Act, then that information will be redacted. A cost estimate will be provided if the charge exceeds $40.00. The city has up to 10 business days to respond to the requestor.

EMS Report Request

Use this form to request a medical report on an individual treated by Helotes Fire & EMS. Billing requests should be directed to the city’s billing agency, Emergicon Emergency Medical Billing via www.chartswap.com.

Police Report Request

Use this form to request an accident (crash) report or incident report within the Helotes city limits.

Patrol By Request

Use this form to request a police officer to drive by your residence during an extended absence. This public service is for residences within the Helotes city limits and requests should be submitted at least 48 hours in advance of the requested time frame.

Pet Licensing

Use this form if you live within the Helotes city limits and need to register your dog and cat over four (4) months of age. Owners will receive a registration tag from the city.
